Enroll in UCare Connect (SNBC)
You have two options for enrollment:
Enroll online
You'll need your Medicaid ID number and an email address to get a validation code.
You can save your progress and finish the application later. Access your in‑progress application through a link in the original validation email.
Enroll by mail
Download and print the enrollment form.
Fill it out and mail it to:
