Need help paying for your plan?

You can apply on MNsure for help to lower the cost of your monthly plan premiums and out-of-pocket costs. Many Minnesotans who enroll through MNsure qualify for financial help, including:

Individual and family plans: Is health coverage you purchase on your own, if you don’t have access through an employer or don’t qualify for government programs, like Medicaid or Medicare. Subsidies are available through MNsure to help pay for your plan.
MinnesotaCare: Pays for medical services with members paying a monthly premium based on income. See if you qualify.
Medical Assistance (Medicaid): Pays for most covered healthcare costs for eligible people. See if you qualify.

If you are over 65 and need help with heath care costs

UCare offers two plans designed for people ages 65 and older who need financial assistance.

UCare’s Minnesota Senior Health Options (MSHO) (HMO-D-SNP): Designed for people who are eligible for both Medical Assistance and Medicare
Minnesota Senior Care Plus (MSC+): Created for people who qualify for Medical Assistance through their county, but do not have Medicare

UCare also offers UCare Medicare Plans that do not include Medical Assistance.

If you have a certified disability

UCare offers two plans designed for people age 18 – 64 with certified disabilities.

UCare Connect + Medicare (HMO D-SNP): Created for people who want to combine their Medical Assistance and Medicare in one plan
UCare Connect (Special Needs BasicCare): Created for people who have Medical Assistance, but may or may not have Medicare

If you or your loved one are in a long-term care facility

UCare offers two plans designed for Medicare-eligible adults who are receiving or are eligible for a nursing-home level of care. These Institutional Special Needs Plans (HMO I-SNP) are offered at two premium and cost sharing levels: UCare Advocate Choice and UCare Advocate Plus.

Both plans offer a team-based, in-facility approach to care with a dedicated care coordinator and primary care doctor. Members must live in a participating facility within the 14-county service area.
